Perform the following actions to unlock additional modes.

Unlockable:How
Hardcore Difficulty (corresponds to the old Insane difficulty setting)Clear the campaign on any difficulty to unlock hardcore difficulty. This setting is equivalent to Zealot difficulty, but only allows you to restart from the last save game. To frustrate matters, hardcore difficulty only allows you three saves (for the whole campaign) and cannot be started from a New Game (+) -- you must start fresh from a New Game. Good luck!
New Game + (visible on the menu, but cannot be selected)Clear the game on any difficulty and you may saved the cleared game data for New Game (+). From there, all of your weapons and items in the store and your inventory are carried over from your previous game. Makes completing Zealot and Hard difficulties much easier!

there are a lot of WESTERN games that are banned in Japan, this means that no store can sell them but they can be sold online. Amazon Japan sells all the banned games(Dead Space, Mortal Kombat)

@ reviniente - I don't believe playable characters can be dismembered. Not sure if this is by law, but perhaps they didn't want to censor this game so they didn't release it in Japan..
