Hatred Gets Adults Only Rating - News

The Adults Rating is almost a death sentence for a game, because Microsoft, Nintendo and Sony do not allow those games on their consoles. Also a lot of retailers will not sell the games. However, there is no policy on Adults Only games on Steam.
"Based on the content depicted in Hatred and the overall context of the game, ESRB assigned an AO (Adults Only) rating with content descriptors that include Intense Violence, Blood and Gore, and Strong Language," an ESRB representative told GamesIndustry.

Its because he is killing innocent people cause he is mentally unstable, which should be an issue not talked about cause people dont like talking about that. Now if you are killing innocents for money or war or drugs or for fun its different.
